2. Fill out Medical Questionnaire online

When completing Health History Questionnaire online, we recommend using either a desktop computer or a laptop and Google Chrome browser in order to ensure the form is submitted correctly. If completed using a cell phone or a tablet, or MAC devices, the form may not go through. If you do not have access to a desktop computer or a laptop, feel free to print out the medical questionnaire in a PDF format and complete it manually, you can download it here, or you can fill it out on your device using Adobe Acrobat Reader. Make sure to save the PDF form via Save As feature before closing the file in order not to lose the data. Email us completed form at info@golightbariatrics.com or fax it at +1-855-457-1400.

3. Schedule a free consultation with the surgeon by phone or video call

Once we receive your health history questionnaire, our patient coordinator will contact you via email within 24 hours (excluding weekends) to set a date and a time frame for a free phone or video consultation with one of our surgeons. Please be sure to review your spam folder, as our emails may be filtered into your email spam.

4. Choose your preferred surgeon and facility

A consultation for evaluation and approval purposes will be scheduled with one of our bariatric surgeons who assist us with pre- and post-op consultations, however each patient chooses their preferred surgeon for the actual procedure during the scheduling process: Dr. Cynthia Hernandez at the NEWCITY Hospital or Dr. Francisco Zavalza at the CIBA Surgical Center.

5. Set your surgery date

After the consultation, your patient coordinator will email detailed scheduling instructions. We will set a surgery date based on your preferences and the surgeon’s and hospital availability. You will have to allow sufficient time for preparation for surgery by following a low-carb protein pre-op diet for a number of weeks depending on your BMI. We limit the number of surgeries to 7 per day for Dr. Zavalza (on average 4 gastric sleeves + 3 gastric bypasses), and 5 per day for Dr. Hernandez (on average 3 gastric sleeves + 2 gastric bypasses).

6. Make a $500 USD deposit to secure surgery date

Deposit of $500 is required to guarantee your operation room, hotel and transportation reservations. The deposit is part of the total surgery cost. 7. Book a flight (or if driving, a parking spot)

Before planning your trip, review our detailed Travel Instructions in order to know for how many days to plan your stay, where to travel to, at what time to arrive and depart, and see our recommended packing list.

8. Sign the Financial Consent Form online

Please read very carefully the Financial Consent form as it is a legal agreement between the Patient and the Go Light Bariatrics facilitating team. You will need to initial and sign the form at https://www.golightbariatrics.com/patient-financial-agreement-consent-and- waiver/ Upon admission to the hospital, you will be asked to sign a separate hospital consent form, that is an agreement between the Patient and the Hospital.

9. Start following low-carb high-protein pre-op diet

In order to decrease surgery complication risks and recovery time, good preparation is required. Please carefully review our Pre-Op DietPre-Op Instructions, and Vitamin List, and follow accordingly. During the pre-op consultation, our surgeon will let you know if any changes to our standard preparation are needed. The length of the diet is based on the BMI (Body Mass Index), and the chart with recommended time frames can be found in the pre-op diet instructions. Calculate your BMI here.

10. Arrive in San Diego / Tijuana for surgery

At least 3 days before your arrival in San Diego, CA (or in Tijuana, Mexico if you are traveling directly), detailed information about your hotel and transportation arrangements will be emailed to you by your patient coordinator. You will be explained where the driver will meet you, how he or she will verify your identity, and what to expect during your stay in Tijuana.
