REFUND POLICY FOR WEIGHT LOSS SURGERY
FOR GO LIGHT BARIATRICS, LLC
Deposit Refund Policy*
All cancellations must be received by Go Light Bariatrics, LLC in writing, email or via fax.
Once the surgery deposit has been paid, 50% (or $250 USD) of the deposit is non-refundable.
If you cancel your weight loss surgery MORE THAN SEVEN (7) days prior to the scheduled surgery date, we will refund FIFTY PERCENT (50%) of the initial deposit*. However, if any other additional expenses are incurred by Go Light Bariatrics, then these expenses will be deducted as well.
For example:
Surgery date January 20th
- Cancellation requested on Jan 12th or earlier will allow for a 50% deposit refund (that is more than 7 days prior to the scheduled surgery date: Jan 20th – 7 = Jan 13th, any day prior to Jan 13th is more than 7 days prior to the surgery date).
If you cancel your surgery WITHIN SEVEN (7) days prior to the scheduled surgery date, NO REFUND of any portion of the deposit will be made*.
- Cancellation requested on Jan 13th or later will forfeit 100% of the deposit (that is within 7 days prior to the scheduled surgery date: Jan 20th – 7 = Jan 13th, any day starting with Jan 13th and further is within 7 days prior to the surgery date).
IF YOU NEED TO POSTPONE AND LATER CANCEL SURGERY (for any reason)
The day when request to postpone surgery has been received will be considered the day when a cancellation has been requested.
For example:
Surgery date January 20th
Request to postpone received on Jan 15th
1. If surgery is later rescheduled and takes place, no portion of the deposit will be forfeited. It will apply in full toward the new surgery date.
2. If surgery is later cancelled, Jan 15th will be considered the cancellation date. No portion of the deposit will be refunded, as such cancellation took place within 7 days prior to surgery date.
Surgery date January 20th.
Request to postpone received on Jan 10th.
1. If surgery is later rescheduled and takes place, no portion of the deposit will be forfeited. It will apply in full toward the new surgery date.
2. If surgery is later cancelled, Jan 10th will be considered the cancellation date. Fifty percent (50%) of the deposit will be refunded, as such cancellation took place more than 7 days prior to surgery date.
SURGERY DATE CHANGE POLICY*
If you need to change your surgery date, and we receive your written request, there will be no additional charge and no loss of any part of your deposit, provided you proceed with the surgery after such change.
Surgery date changes may be requested 2 times at no additional cost.
If you need to change the surgery date more than twice, an additional fee of $100 USD will apply for each such change and will be charged before the new surgery reservation is confirmed.
*Deposit of $500 USD is required to secure a surgery reservation. If you decide to make a deposit of more than $500 USD, the amount that exceeds $500 USD will be fully (100%) refunded, and the above-described policy will apply to the $500 USD portion of your deposit.
In case of unforeseen circumstances such as but not limited to pandemics, weather conditions, war, terrorist attacks, etc., that would put our patients’ health and well-being at risk, we may be forced to change surgery dates. In such situations, a patient will be given 2 options.
In case such situations as mentioned above affect our services described in this consent form, and we must temporarily change or limit certain services in order to protect our patients’ health and well-being, a patient by signing this form understands and agrees to such changes.
SURGERY CANCELLATION BY SURGEON POLICY*
If you arrive in Tijuana, Mexico, for surgery and the Surgeon decides, for your health and safety, not to attempt or complete your surgery, you will be responsible for all expenses including but not limited to, transportation costs, hotel stay costs, lab fees, etc., incurred because of the cancellation. Upon said cancellation, 50% of your initial deposit will be refunded to you after all expenses incurred by you are repaid.
Airline tickets purchased by clients or for clients are non-refundable
Any cancellation that is the result of a death or serious illness will be handled on a case by-case-basis.
SURGERY CANCELLATION DUE TO THE PATIENT NOT FOLLOWING PRE-OP INSTRUCTIONS OR BEHAVING INDECENTLY
If you arrive in Tijuana, Mexico, for surgery and the medical team learns that you haven’t been following the pre-op instructions and this may significantly increase post-op complication risks and put your health or life in danger, the surgery will be cancelled. In that case you will be responsible for all expenses including but not limited to transportation costs, hotel stay costs, lab fees, etc., incurred because of the cancellation. Upon said cancellation, the $500 initial deposit will be forfeited because such cancellation is caused by the patient, the remaining surgery balance payment will be refunded to you after all expenses incurred by you are repaid.
Examples of not following the pre-op instructions that may lead to surgery cancellation include but are not limited to: not discontinued use of NSAIDs 7 days pre-op (unless specifically instructed by the medical team in writing not to stop NSAIDs), not discontinued use of street drugs, not discontinued or significantly reduced smoking, not following the pre-op diet as instructed, not discontinued consumption of alcohol at least 7 days pre-op, etc.
The same policy will apply to the patients that choose to behave indecently toward any of the facilitating or medical staff, using foul language or being violent, or refusing to follow the hotel, hospital or company rules. Such patients or their companions will be asked to leave at their own expense, their deposit will be forfeited, and the surgery balance will be refunded after the incurred expenses have been repaid.
CREDIT AND DEBIT CARD POLICY
All credit card and debit card processing fees charged are non-refundable at any time.
